Please answer the following questions:

1. Do you live in the state of Indiana?

2. Was your student between the ages of 5 and 22 by August 1st, 2022?

In order to verify your student’s eligibility, we have just a few more questions to ask to see if you might qualify based on specific tracks as determined by the State of Indiana Department of Education.

1. Has your child attended a public school (K-12) in the state of Indiana for at least two consecutive complete semesters prior to the current semester?

2. Does your child currently attend a private Catholic or charter school?

2(a). Did your student receive a Choice Scholarship (voucher) in any previous School year? (Student MUST have completed an entire academic year at said school.)

3. Has your student ever received a scholarship from a Scholarship Granting Organization (SGO) for any previous school year?

3(a). Did your student receive the Indiana Choice Scholarship in the 2020-2021 school year, but withdraw from that school before the end of the academic year?

4. An SGO is a non-profit 501(c)(3) organization that receives contributions from donors and grants educational scholarships to eligible students. Insurance companies that contribute to SGOs participating in the program can receive a tax credit. Has your student ever received a scholarship from a Scholarship Granting Organization (SGO) for any previous school year?

4. Does your student have a disability that requires education services such as an Individualized Education Program (IEP), pursuant to IC 20-32, or a Service Plan (SP), pursuant to 511 IAC 7-34?

4(a). Does your student have an Individualized Education Program (IEP), pursuant to IC 20-32, or a Service Plan (SP), pursuant to 511 IAC 7-34?

5. Based upon your current residence, has your child been assigned to attend an “F-rated” public school for the next school year?

6. Does your student have a sibling who has received either a (1) Indiana Choice Scholarship or (2) SGO scholarship of at least $500 for any school year?

7. Did your student receive and use an Early Education Grant (On My Way Pre-K Program) to attend Pre-K at an eligible Choice school?

7(a). Is your student applying for an Indiana Choice Scholarship at the same Choice Scholarship school they attended Pre-K with an Early Education Grant?

GREAT! Based on the information you provided in the previous screens, your student(s) may be eligible to receive the Indiana Choice Scholarship. We have just a few more questions to determine their eligibility based on household income.

Unfortunately, based on the information you entered, your student would not qualify for an Indiana Choice Scholarship (voucher). However, if you may be eligible for an SGO tax credit award that will allow your student to receive the voucher after one year in one of our Catholic schools. Please provide answers to the following questions:
